When you visit Dr. Dickerson's office you will need to fill out our Patient Information Form. This form gives us the information we need to meet your orthodontic needs safely and efficiently. You will need to have information regarding your employment and insurance information as well as your dental and medical history. For your convenience we have provided the Patient Information Form online. You can fill it out online or download the pdf and print the form so you may bring it in when you visit our office.
